When making an appointment with the Tioga County ACCORD Petition Intake Staff please keep in mind the following:

  • Arrive for your appointment 10 minutes before your appointment time in case the Intake Staff has any additional questions.
  • If you are unable to keep your appointment time, please contact the Family Court office as early as possible to cancel or reschedule. Appointment times fill quickly and your appointment time may help another client.
  • When making your appointment, you will need to provide your name, your phone number, the name of the person you are filing against, the names of the children's parents, the names of the children involved, and the type of petition you wish to file (Petition Intake Staff can assist you in determining the type of petition)
  • You will need a current address for the party or parties against whom you are filing. Family Court will not locate this for you, and your petition will be dismissed without current addresses.
  • Bring your drive's license or non-driver's picture ID so your paperwork can be notarized.

For Support petitions - child or spousal: You will need to fill out a financial disclosure form (Financial Disclosure Affidavit) and attach, at a minimum, your last filed income tax return with W-2 forms, and most recent pay stub. If you cannot submit any of these documents, you will need to provide a written explanation with your petition indicating why that document cannot be filed. Please remember to bring proof of your current income, and copies of your last filed state and federal tax returns with W-2 forms to your scheduled appointment.

For Original Custody/Visitation and Child Support petitions: If the child's parents were not married when the child was born, you must attach proof of paternity (i.e. the Acknowledgment of Paternity form or the Order of Filiation) to your petition or it may be dismissed.

For Modification or Enforcement petitions regarding an order from another court: You must bring the relevant order to your appointment, and a copy must be attached to the petition.
